I, Christian Alexander Dorsett, having the capacity to make health care decisions, willfully, and voluntarily make known my desire that my dying shall not be artificially prolonged under the circumstances set forth below, and do hereby declare that:

I understand by using this form that a terminal condition means an incurable and irreversible condition caused by injury, disease, or illness, that would within reasonable medical judgment cause death within a reasonable period of time in accordance with accepted medical standards, and where the application of life-sustaining treatment would serve only to prolong the process of dying. I further understand in using this form that a permanent unconscious condition means an incurable and irreversible condition in which I am medically assessed within reasonable medical judgment as having no reasonable probability of recovery from an irreversible coma or a persistent vegetative state.

In the absence of my ability to give directions regarding the use of such life-sustaining treatment, it is my intention that this directive shall be honored by my family and physician(s) as the final expression of my legal right to accept or refuse medical or surgical treatment and I accept the consequences of such decision.

If another person is appointed to make these decisions for me, whether through a durable power of attorney or otherwise, I request that the person be guided by this directive and any other clear expressions of my desires.

If I am diagnosed to be in a terminal condition or in a permanent unconscious condition I **DO** want to have artificially provided nutrition and hydration, and to be equipped with and be able to interact through a personal human brain-computer interface to discuss my own condition in some manner, until such time as my brother Zachary confers orally and in writing with my attending physician(s), and thereby being fully informed my condition is terminal and/or in a permanent unconscious condition and where the application of life-sustaining treatment would serve only to artificially prolong the process of my dying, and that I am not able to respond to or interact with others when I am using any brain-computer interface, and that if Zachary directs that such treatment be withheld or withdrawn, that I be permitted to die naturally. The Elusive Theory of Everything

Model-dependent realism: the idea that a physical theory or world picture is a model (generally of a mathematical nature) and a set of rules that connect the elements of the model to observations.

According to model-dependent realism, it is pointless to ask whether a model is real, only whether it agrees with observation.

If two models agree with observation, neither one can be considered more real than the other.

A person can use whichever model is more convenient in the situation under consideration.

Each theory may have its own version of reality, but according to model-dependent realism, that diversity is acceptable, and none of the versions can be said to be more real than any other.
